Surrogate decision-making based on “substituted judgment” is the concept where surrogates choose the treatment the patient would most want to receive when the patient becomes incompetent.1 The surrogate decision-making process assumes that the surrogate understands the patient’s values and beliefs and uses them to derive the patient’s end-of-life care preferences.
